The undersigned purchaser agrees to pay, in the event his/her account becomes delinquent and is turned
over to an attorney for collection, reasonable attorney's fees plus all court and attendant collection costs.
I/We agree that all invoices past due are automatically assessed service charges of 1.5% per month (18%
annual rate) and become part of the total balance owing. Walker's Office Supplies, Inc. reserves the right
to hold orders if your account goes beyond credit terms.
I hereby authorize release of any credit information from any source, to Walker's Office Supplies, Inc. for
